DENTAL FEES & INSURANCE INFORMATION
For the convenience of our patients, Kings Road Family Dentistry (Dr. Patrick J.S. Finnigan Inc.) will accept payment directly from your insurance company (assignment) under the following circumstances:
All details and information regarding your dental plan is provided to us by you, and is kept up to date. It is your responsibility to advise us of any changes to your coverage immediately, such as layoff, change of employment, marital status, etc. Uninsured balances are paid in full on the date of service.
Insured balances are paid within 90 days of date of service. You will be notified of your dental insurance provider inactivity 60 days after date of service. If, after 90 days, your dental insurance provider has not responded with your payment, you are personally required to pay the balance of your account. Please be aware, that by this point, we will have reviewed and contacted your insurance company on your behalf, at least three times.
We are happy to provide you with completed dental claim forms so that you may claim your benefit directly from your employer or dental insurance provider (non-assignment). Payments can be made by cash, cheque, debit, MasterCard, Visa, and American Express, and coming soon, e-Transfer.
We understand that dental insurance is an important part of the cost of dental care. It is in your best interest, should questions arise, to discuss with your employer the full extent of your benefits. We will do everything possible to help you receive the insurance benefits to which you are entitled; however, we cannot take responsibility for your dental insurance provider’s non-payment of an agreed upon and completed treatment.
Please note that we require 48 hours’ notice to cancel or reschedule an appointment at no charge. If adequate notice is not given, there will be a charge of $125 for appointments of one hour or less and $200 for appointments scheduled for more than one hour. You will be informed of the length of appointments at the time of booking.
